Each student nurse will work with a partner (“the patient”) to complete a health history interview using Gordon’s Functional Health Patterns to document a narrative overview of the patient’s health. The “patient” will be free to use their own personal information or may answer the questions in the interview acting as someone else (the “patient” does not need to disclose if the information is real or is part of acting as a patient). Student will follow the Functional Health Pattern Rubric, along with text or online resources for Gordon’s Functional Health Patterns and Erikson’s stages of development to complete the assignment.

The assignment is to be completed in narrative format, with an average of one paragraph for each of the required areas on the assignment rubric. Students will be graded on their attention to detail, protection of patient identity, professional APA writing style and lack of opinion during the assessment overview. Cover page and references are required.

NTC Program Outcome Assignment. Student must demonstrate minimum of 80% proficiency on this assignment. Students who do not perform at 80% will be required to resubmit assignment and meet proficiency before grade is released to registrar.
