Select a population within your practice community.

Using links in this week’s learning resources locate state, county health, or zip code data to identify and describe a significant health or health-related problem for the population you have selected within your practice community. NOTE: For your initial discussion post you are required to insert the ‘link’ to the exact page you sourced the data you utilized to identify the significant health or health-related problem for the population.

Links from the learning resources are added here as a convenience for you to be able to readily access them.

As you are searching, surveying and synthesizing this data keep in mind that in future assignments you will be required to develop a population health intervention to address the significant health or health related problem you have identified.

During your search you may locate more than one significant health or health related problem. For purposes of this unit discussion you will need to narrow the discussion to a single problem that you will be able to successfully develop the three part population health intervention assignments.

Below is a link the “Wheel of Intervention” that provides guidance on what constitutes a population health intervention.
