Disability Care Direct (DCD) has its policies and procedures that describe the Agency’s plan for monitoring the presence of Basic Assurance. Agency makes use of essential tools such as “probes” to assist in determining what works and what does not work. There must be information that include whatever is necessary to answer the questions fairly and accurately. Some strategies for information gathering include but not limited to:

 

· Holding small group meetings

· Making observation and Site Visits

· Interviews with key staff and people supported

· Review of written materials

· Policies and Procedures

· Mission/Vision Statements

· Person-Centered Service Plans

· Meeting and Committee Minutes

· Personnel Files

· Critical Incident Reports, Consumer Complaints, etc.

 

Agency utilizes organized and coordinated methods and the systems are the structure. These can include, but are not limited to:

 

· The Mission, Goals and Objectives

· Policies and Procedures

· Job Descriptions and Duties Performed

· Methods and Tools (Training, Committees, Resources, etc.)

· People (skills, leadership, HR functions, etc.)

· Organizational Structure, Chart and Hierarchy
