Based on your assessment of the issue at the facility that you identified in XC4001, propose a practice change to address one issue related to population health or quality and safety at your practicum site. The scope of the change should be limited to actions nurses can take; 
it should be a change that is under nurses’ control. Your change should be based on Lewin’s change management theory.
