After learning about social determinants of health and gathering your county research, you will start working on preparing your presentation.

  • Prepare: Your final project should be prepared in a PowerPoint “type” format (PowerPoint, Prezi, etc.) with either voice-over or video presentation. Items in the presentation should include (but is not limited to):
    • Analysis of statistical data relating to county population (approx. 2-3 slides)
    • SDH strengths and needs in the county and how that relates to Healthy People 2030 goals (at least 1 slide for each)
    • Available county resources to address the SDH needs and ways to assist clients to accessing those resources (approx. 2-3 slides)
    • Incorporation of your patient profile
  • Presentation: Your final SDH presentation will be posted in the discussion below. Your presentation should be creative and include appropriate pictures or graphics. The presentation should be a maximum of 12-15 slides and 10-15 mins worth of presentation (video/voice-over) time.
    • It is recommended that you visit the assigned county for an immersive experience and gather real-life images to add to your presentation (be mindful of social distancing and personal privacy ie. no pictures of people without written permission)
