A lead pharmacist has noticed that pharmacy technicians are occasionally shorting prescriptions by 1 or 2 pills. The technicians have been at the pharmacy for 3 or more years and are generally high performers, but the lead pharmacist attributes the decrease in efficiency to the fact that technicians are working longer than typical hours, which is a result of short staffing. Since the two highest performing technicians are returning to work from vacation the following week, the lead pharmacist sees the problem as temporary and wants to avoid a time-consuming refresher training. She resolves the problem by reviewing the prescriptions filled by technicians herself as often as she can, hoping to reduce the errors as much as possible.

Consider the following scenario:

Tim has been hired to lead the research department of a large healthcare organization. The organization’s CEO has asked Tim to roll out new goals and reorganize teams to reach desired organizational objectives. Tim has some new ideas for the organization and immediately begins implementing a new strategy for marketing the desired goals and the new team reorganization. Relying on his perspective as an agent of change from the outside, Tim doesn’t take the time to get to know team members and their individual strengths, to inquire into how the department is currently run, or to familiarize himself with the current trends. He also has not set any targets for the goals and does not communicate his plan. Upon launch, the goals are not well-accepted by the team, who understandably feel that they were not consulted. The reorganization of team members has left some members angry, with one researcher threatening to leave the company.

The board of a healthcare clinic is looking for ways to bring in more revenue. The Chief Financial Officer (CFO) proposes to raise prices. However, the population the clinic serves is, on average, beneath the poverty line, and many are uninsured. The CFO argues that if the prices are raised, the additional revenue would offset any losses the clinic might suffer as a result of the price increases and loss of patients. The staff of the clinic is adamantly opposed to this idea, but their views are not being considered by the board.

Roxy, the clinic’s care manager, is tasked with motivating, informing, and inspiring the staff in new measures and policies aimed at increasing the quality of care. Roxy is convinced that staff are moving too quickly and not spending enough time with each patient. She knows this is a result of unavoidable time constraints but wishes to implement practices that give healthcare professionals more choice in the matter, to address a patient’s needs. She must balance this with the organization’s mandate to maximize revenue by serving as many patients as can be treated and by maximizing the human resources within the clinic.
