Medications (write out a complete order, even for OTC meds); 3.) Suggested consults/referrals; 4.) Patient education; 5.) Follow-up, including time interval and specific symptoms to prompt a return visit sooner; 6.) Provide rationales for each intervention and include references to support your plan. Clinical practice guidelines should be utilized as applicable. (Evidence of Academic Integrity Issues will result in a grade of 0.)

HPI:
A 26 yrs old female presented at the office today with worsening headaches accompanied by nausea and vomiting. The patient has had a significant history of headache for the past 10yrs but has been having an increased frequency of headache and worsening symptoms. Headache are located behind the left eye, throbbing pain with an 8-10 in intensity and an aura of light and noise. A dark room relieves her symptoms and sleep. She has tried Acetaminophen and ibuprofen with minimal relief. She has a family history of headache (mother). Physical examination: normal Neurological exam, cranial nerve exam intact with no neurological deficit.
