Your goal is to succinctly describe your plan in 1,400 to 1,750 words, not including the title page or abstract. Include the following sections:

Abstract: Provide an abstract of approximately 350 words that summarizes your program proposal.
Background and Need: Use the literature review you previously developed and expanded upon to provide a concise background about the community health issue you are addressing.
Program Plan: Use your detailed outline to develop a narrative description of your intervention by program planning phases and steps. Integrate other relevant material you have worked on, including your logic model or flow chart.
