      Wk 6 – Course Capstone Project Final Proposal Submission

      Assignment Instructions

      Compile all your research into a single program proposal, drawing from the literature review, methodology, intervention description, and outline you have already completed.

      Your goal is to succinctly describe your plan in 1,400 to 1,750 words, not including the title page or abstract. Include the following sections:

      • Program Title
      • Abstract: Provide an abstract of approximately 350 words that summarizes your program proposal.
      • Background and Need: Use the literature review you previously developed and expanded upon to provide a concise background about the community health issue you are addressing.
      • Program Plan: Use your detailed outline to develop a narrative description of your intervention by program planning phases and steps. Integrate other relevant material you have worked on, including your logic model or flow chart.

      Cite your sources and format your assignment according to APA guidelines.
