Problem Statement:

Between 1/1/2024-12/31/2024, Bison Hospital has experienced a high rate of hospital readmission among patients age 65 and older with chronic condition of congestive heart failure. Data indicates that readmission rates are approximately 29%, which exceeds the national average of 25%. These preventable hospitalizations not only impact patient health outcomes but significantly increase the cost of care. Efforts are needed to identify the root causes and implement solutions to reduce avoidable admissions.
