As a reminder, each week, your assignment will comprise a vital component of your Final Community Stakeholder Presentation and Final Community Assessment Report final projects. As such, you are encouraged to ensure that you use the feedback you receive each week to make the necessary revisions for your final report.

It is recommended you review the logic model examples. Logic models are very important to the evaluation process. They help to provide a clear picture of the different components of the program and how they connect to your action plan.

In your paper,

· Create a logic model for each action plan (1 is allowed if it is comprehensive).

· Create a logic model diagram for your community needs and action plans. You can attempt to develop one model that addresses both action plans or two models for each if they are completely unrelated (e.g., reducing drug addiction and repairing sidewalks).

· Utilize cells and connection arrows as shown in the

· Remember to create individual cells for each asset, input, output, intermediate outcome, and ultimate outcome. Once the cells are created, use arrows or lines to show how those cells are connected.

· Graph all components (asset, input, output, intermediate outcomes, and ultimate outcomes).

. Specifically, p. 10 shows a guide for creating your logic model diagram. If you use Microsoft Word or PowerPoint, these designs are available under “Insert” and then “SmartArt.”

· Explain each of the components of the logic model via ScreenPal.

· Develop a script once the logic model is made that addresses the components of the video presentation outlined in here.

· Select a screencast program. For assistance, you may review the following:

· Create a video presentation (5 to 7 minutes of at least six slides).

· Describe the rationale behind the components you have included in your logic model diagram.

· Evaluate how the logic model connects to your community and action plans.

· Evaluate the effectiveness of the intended model outcomes in addressing your community’s greatest needs and how they will impact the proposed target population.

· Assess how using a logic model will help in your action plans and stakeholder pitch presentation.

Submission Requirements

You are required to

· Submit a script of your screencast narration.

· Your script should be at least two pages in length in a typed Word document. Please use Slide 1, Slide 2, and so forth headings to provide clarity. Make certain to include a working URL to your screencast at the top of the page.

· Upload the assignment to both Waypoint and Folio adhering to the following requirements:

· You will need to access your Folio account to upload your assignment to your ePortfolio.

· After you upload your assignment to your ePortfolio, you will need to export the file as a PDF to be able to submit your assignment to Waypoint for grading.

· must include a one- to two-page Word document and formatted according to

 resource. You must submit this document to WayPoint with the following:

· the active URL link to your video (or narrated PowerPoint)

· your list of references formatted in APA Style

· must use a screencast program (Jing, Screencast-O-Matic, PowerPoint, etc.) to create a video presentation addressing the outlined directives.

· must include a video submission five to seven minutes in length and containing at least six slides.

· must use data, pictures, graphics, and other visual components to help support your content. The required tables do not count toward this component.
