Topic I began caring for a 66-year-old patient who required three subcutaneous epoetin alfa (Procrit) injections each week. I performed extensive initial examinations to determine the patient’s health status and set a baseline for their treatment. My primary focus was on proper pharmaceutical administration and patient education, ensuring that they knew the treatment’s goal and any negative effects. A key obstacle was the patient’s fear of self-injections, which I addressed with extensive demonstrations and helpful practice sessions. This trimester focused on patient education and trust-building, as well as improving my communication skills and encouraging patient compliance.The second trimester consisted of monitoring the patient’s response to treatment and making appropriate modifications. I used critical thinking to analyze lab results and worked with the patient’s healthcare physician to ensure the best possible care. The patient had modest side effects, which I minimized by changing injection sites and advising fluids and relaxation. This session emphasized the importance of ongoing assessment and adaptive treatment plans, which improved my capacity to foresee problems and execute proactive actions to improve patient outcomes. I assessed the treatment’s efficacy and planned for future care. The patient’s hemoglobin levels had significantly improved, indicating a good response. Preparing the patient for a more self-managed care approach was difficult, but was accomplished by developing a complete, detailed care plan. This trimester underlined the significance of holistic care and rigorous discharge planning, which improved my skills in designing sustainable care plans that empower patients and families.
