For your final paper, please answer the questions below as they are numbered here, retyping the question (typed below in bold) before answering it.

1.
Did you revise your short-term (end of the quarter) goal in any way? If so, why and how? (Be brief and clear.)

· One paragraph should be plenty for this question

2.
Did you achieve any progress toward your goal (original or revised)? If so, exactly what was accomplished and how do you know? (Again, be clear and stick to the question.)

· A couple of paragraphs should be plenty for this question

· Be sure to mention your monitoring results

· You should attach a one-to-two-page sample or summary of your monitoring sheets (no more than two pages, please). Also, mention whether you earned or denied yourself rewards and what

3.
Why were you able/not able to be successful (or more successful)?

· Analyze your behavior change project using course material (two to three pages for this)

· Use two to three concepts from the text (Chapter 3) and the class lectures to apply theory and research to your problem. For example, you might indicate what stage you were in when you started the project using the Transtheoretical Model (Prochaska and DiClemente), and possibly describe what stage you are in now and why. If you used specific approaches described in the text, point this out explicitly. Explain how you used them and indicate whether they worked

· If you did not use approaches from the text, explain how you could have and how it might have improved this project. You may also draw from Chapters 4 and 5 in the text on behavior change approached and research findings for your specific behavior (exercise, weight loss, smoking cessation) if they apply

· Tie in the material very carefully and concretely to your project. In what ways did your project use or fail to use these principles?

See text for: Theory of Planned Behavior, cognitive behavior change approaches, stimulus control, self-reinforcement, contingency contracting, behavioral assignments, relapse prevention, stages of behavior change, Transtheoretical Model of behavior change

This analysis should involve careful thinking leading to summarizing your best thoughts succinctly. Use your space wisely.

4.
Will you continue this project or not? Briefly describe why or why not, and what you got out of the project. (One paragraph)

Remember

· Attach your graded original behavior change paper (and revised one if one was done)

· Attach a one-to-two-page sample or summary of your monitoring results

· Plan your analysis carefully as if it were a take-home essay question

· Double-space, 12-point font, one-inch margins, no typos, no cover page needed
